L’Chaim Health Center
Since 1999, our L’Chaim Adult Day Health Center has provided outstanding medical, rehabilitative, and social services to thousands of elderly and disabled clients.
At L’Chaim, we offer a variety of services under one roof, specially tailored to meet individuals’ and families’ needs in a warm, supportive, and friendly atmosphere.
Our staff of professionals provides nursing and personal care; case management; physical, occupational, massage, and speech therapy; nutritional counseling; and recreational and socialization activities. Transportation, hot lunch, and snacks are also provided. Our goals are to maintain and enhance our clients’ health; help them to remain independent in their homes and community for as long as possible; and reduce the likelihood of falling, emergency room visits, hospital stays, or premature institutional care.
Building Community
Center hours are 9:00 am – 1:00pm and 2:30 – 6:30pm, Monday through Friday. Staff is available 8:30am – 7:00pm on those days.
Scholarships and subsidies are available and we accept Medi-Cal participants.
L’Chaim, located at 2534 Judah Street, is in the heart of San Francisco’s Sunset District. Our staff is bilingual in Russian and English.
