Rehabilitation Aide

L’Chaim Adult Day Health Center is a licensed facility and outpatient center that provides medical, social, and therapy services for Russian-speaking older adults and individuals with disabilities. We have been a haven of person-centered care and community support for thousands of emigre clients from San Francisco and San Mateo for over 25 years.
POSITION SUMMARY:
Under the supervision of the Nurse Supervisor II and in collaboration with licensed rehabilitation staff, the Rehabilitation Aide performs Aide supports the delivery of restorative and rehabilitative services to program participants in accordance with applicable Title 22 regulations and program requirements. The Rehabilitation Aide assists with implementing assigned Physical Therapy (PT) and Occupational Therapy (OT) activities and exercises as directed by licensed therapists and within the scope of the position. The Rehabilitation Aide may provide language translation assistance to facilitate communication between participants and licensed rehabilitation specialists and assists with maintaining accurate and timely documentation in TurboTAR and other agency systems as assigned. The Rehabilitation Aide helps maintain a safe, organized, and participant-centered environment that promotes dignity, safety, independence, and quality of care in compliance with Title 22 Adult Day Health Care standards, confidentiality requirements, infection prevention protocols and agency policies.

Pay Range: $23.50/hr (depending on experience)

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
- Under the direction and supervision of the licensed Physical Therapist (PT), Occupational Therapist (OT), or Registered Nurse (RN), assists with implementing assigned restorative or maintenance therapy activities, exercised, and non-skilled treatment modalities in accordance with participant care plans and program policies
- Assists with conducting structured exercise and restorative maintenance groups as directed by licensed rehabilitation or nursing staff
- Assists participants with the safe use of assistive devices, adaptive equipment, walkers, wheelchairs, and related supportive equipment as instructed by licensed staff. Reports equipment concerns, malfunctions, or participant safety issues promptly
- Maintains accurate and timely documentation related to assigned PT and OT activities in accordance with program procedures, Title 22 requirements, and departmental standards, including attendance records, flow sheets, and other assigned documentation
- Under the supervision of licensed staff, assists with implementing maintenance exercise and wellness activities designed to support participant mobility, strength, endurance, and functional independence
- Provides translation and language interpretation assistance for PT, OT, nursing, or program staff as needed to facilitate participant understanding and engagement
- Assists Program Assistants and interdisciplinary staff with participant activities of daily living, meal service, activity programming, participant supervision, transportation support, and other assigned operational needs
- Facilitates basic exercise, stretching, or wellness-focused activity groups for participants within the scope of the position and under appropriate supervision
JOB QUALIFICATIONS:

Russian language conversational skills required

High School Diploma or equivalent required

Experience in adult day health care or similar setting preferred

Ability to communicate effectively with participants, families, caregivers, and staff

Ability to understand written treatment plans

Aptitude for working with older adults

Ability to meet health screening requirements, including TB clearance in accordance with Title 22
