Documentation to Submit
Please submit the attachments described below to [email protected] with subject line―“CTCHGE Grant Report (Organization/Individual’s Name).”
- A list of schools and teachers engaged with contact information for each attending school—USE THIS TEMPLATE
- A FINAL financial report using the provided template, that compares total budget vs. total actuals (expenses and revenue) through the entirety of the grant term (through June 30, 2024). Please include an explanation in the notes column when actual revenues or expenses differ and an itemized list of receipts for the most recent expenses incurred (January – June 2024) by name, date and amount in the receipts column—USE THIS TEMPLATE
- Any other supporting documentation (optional)
Please upload all receipts to your organization’s Final Grant Reporting Folder (Sept 2024) emailed to you.
