How to Use This Form
The Special Needs Trust application form is comprised of 5 pages. Please note there are several required fields on every page, and you must complete all required fields before advancing to the next page.

Requested Documentation
At some point during the application process, you will be required to supply the following documentation:
- Special Needs Trust
- Recent Evaluations or Assessments
- Financial Projection
If you have any or all of these documents now, you may upload them on the final page of this form.
To submit your documents separately, please include them as attachments in an email to Ryan Berman, [email protected], and Rebecca Paul, [email protected], with the subject line―“SNT: Documentation/Individual’s Name”
