MSSP SW Care Manager

Under the supervision of the MSSP Site Director (or their designee) , the MSSP SW Care Manager works as part of an interdisciplinary team to provide home assessment and case management services to clients of the Multipurpose Senior Services Program (MSSP). The Care Manager understands and assists in meeting the mission, service and budget goals of Jewish Family and Children’s Services, and works within the limitations of the Agency’s resources and performs all duties in compliance with agency standards.

Responsibilities

  1. Provides in-depth assessment/reassessment services to functionally impaired seniors and their families, identifying functional limitations that limit independent living, in compliance with MSSP requirements and Agency standards for performance.
  2. Based on the SAH and MSSP Protocols, determines the level of care management services appropriate for clients and provides services in compliance with community and Agency standards.
  3. Develops, implements and monitors care plans for clients, implementing services and providing follow-up and monitoring as described in the care plan.
  4. Identifies and develops support systems for clients, consulting with other members of the interdisciplinary care management team as appropriate.
  5. Provides reassessment services on a regularly scheduled basis.
  6. Refers clients to special consultants to provide detailed evaluations and recommendations to improve functionality, as indicated. Provides consultation and services to caregivers as indicated.
  7. Presents cases to Agency consultants, as appropriate. Represents agency at community meetings and interagency client conferences as requested.
  8. Complies with the Jewish Family and Children’s Services Policy and Procedures Manual as well as agency health and safety standards for clients, employees and the agency.
  9. Understands and complies with all MSSP policies and regulations.
  10. Attends related staff meetings and care plan meetings as assigned.
  11. Proactively maintains the skills and required license to perform job functions.
  12. Undertakes other duties and special projects, such as outreach and advocacy, as may be assigned by the MSSP Site Director, and JFCS leadership.

Required Knowledge, Skills, and Abilities

  • Ability to communicate effectively with clients, their families and other community representatives.
  • Proficient in computer skills: Microsoft Office Word, Excel, Acrobat Reader, Teams. Ability to use electronic health systems to maintain case records and email to communicate with teams and clients.
  • Ability to work effectively as part of a multi-disciplinary team.
  • Desire to work in a social service setting.
  • Must have a reliable mode of transportation to accomplish the essential functions of the job, i.e. client visits and assessments.

Required Education and Experience

  • College degree in counseling health, social work, gerontology, or other related field from an accredited four-year college or university.
  • Master’s Degree in social work, psychology, counseling, rehabilitation, gerontology or sociology is preferred.
  • Minimum of two years’ experience working with seniors or commensurate experience with in-depth knowledge, skills and experience necessary to perform core functions of case management.
  • Previous experience in implementing psychosocial assessments and writing summaries of findings.

This is a part-time, non-exempt, non-benefited position. Competitive compensation will depend on experience and proficiency level.
