POSITION SUMMARY:
Under the supervision of the Program Director, the Nurse Care Manager works as part of an interdisciplinary team to assess the needs of frail elderly referred to Multi-Purpose Senior Services Program (MSSP), develops care plans and assists in implementing plans as needed. The Nurse Care Manager understands the mission, goals, and objectives of MSSP and of JFCS, and works within the limitations and resources of MSSP and JFCS.

JOB TITLE: Multi-Purpose Senior Services Program (MSSP) Nurse Care Manager
REPORTS TO: Program Director
EMPLOYMENT TYPE: 20 hours/week, part-time with no benefits
JOB LOCATION: San Rafael, CA
SALARY RANGE: $50-$55 per hour
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
- Conducts in-depth in-home assessments/reassessments for senior clients
- Certifies level of care determinations for all MSSP clients, based on MSSP protocols
- Participates in the development, implementation and monitoring of care plans for MSSP clients in conjunction with the Social Work Care Managers, and in cooperation with physicians, health care professionals and other service providers involved with the client
- Provides consultation to social work care managers regarding client’s health and medical conditions including medications. Acts as liaison to clients’ medical providers when communication from a nursing professional is warranted
- Establishes eligibility for oral nutritional supplements according to MSSP policy and maintains required documentation for provision to client according to MSSP program requirements
- Ensures timely and accurate completion of case recordings
Requirements
QUALIFICATIONS:
- Current California Department of Consumer Affairs Board of Registered Nursing license, plus a minimum of one year of clinical nursing experience in elder care is required. PHN experience/certification is preferred
- Graduate of an accredited registered nursing program
- Ability to use word processing software, email, and electronic health systems to maintain case records and communicate with staff
- Current Basic Life Support (BLS) certification for health care providers issued by the American Heart Association required
- Must have received or be willing to receive the Covid-19 vaccination by the date of hire to be considered
