Geriatric Care Manager

JOB TITLE: Geriatric Care Manager
REPORTS TO:  Program Director
EMPLOYMENT TYPE: Full-time (0.80) with benefits
JOB LOCATION: Marin County

Under the supervision of the North Bay Regional Director, the Geriatric Care Manager works as part of an interdisciplinary team to provide home assessment and case management services to clients of the Multipurpose Senior Services Program (MSSP).

The Geriatric Care Manager understands and assists in meeting budget and services goals, understands the mission and objectives of Jewish Family and Children’s Services and works within the limitations of the Agency’s resources, and performs all duties in compliance with agency standards.

  • Salary Range: $70,000 – $80,000 (salary range is prorated based on hours hired)


  • Provides in-depth assessment/reassessment services to functionally impaired seniors and their families, identifying functional limitations that limit independent living, in compliance with MSSP requirements and Agency standards for performance
  • Develops, implements, and monitors care plans for clients, implementing services and providing follow-up and monitoring as described in the care plan
  • Identifies and develops support systems for clients, consulting with other members of the interdisciplinary care management team
  • Understands and complies with all MSSP policies and regulations
  • Participates in JFCS and departmental meetings and trainings and represents Multipurpose Senior Services Program (MSSP) department in a professional demeanor
  • Maintains client privacy and confidentiality
  • Fosters a spirit of teamwork and unity among JFCS staff that allows for cultivating ideas, expeditious resolution of conflict, and the appreciation of diversity as well as cohesiveness, supportiveness, and working together to enable each employee and the department to succeed
  • Complies with the Jewish Family and Children’s Services Policy and Procedures Manual, as well as agency health and safety standards for clients, employees, and the facility
  • Performs other duties as assigned by the North Bay Regional Director


  • Graduation from an accredited four-year college or university with a degree in nursing, health, social work, gerontology, or other related field
  • Masters Degree in social work, psychology, counseling, rehabilitation, gerontology or sociology
  • Two years’ experience working with seniors or commensurate experience
  • Intermediate proficiency in Microsoft Word & Outlook
  • Ability to work effectively as part of a multi-disciplinary team
  • Belief in providing services through a partnership with the client
  • Desire to work in a social service setting
