Palliative and End of Life Care

At one of the most difficult times for people, JFCS can provide an interdisciplinary team of professionals who support the lives of those who are failing or dying with a plan and services that address medical, psychosocial, and spiritual needs.

Principles of Palliative Care

The JFCS Palliative and End of Life Care Program is designed to:

  • Relieve suffering and improve quality of life.
  • Respect the goals, needs, and choices of the client
  • Provide assistance with relief of pain and stress at any stage of a serious illness
  • Support optimum quality of life for clients and families
  • Address medical, emotional, social, and spiritual needs
  • Coordinate care among healthcare providers and care settings throughout the course of an illness

Interdisciplinary Team

A coordinated team of professionals and volunteers, including nursing specialists, care managers, clergy, caregivers, and a consulting physician:

  • Works closely with your physician and other healthcare providers
  • Coordinates hospice services if and when chosen by you

JFCS' Palliative and End of Life Care Program is offered under our Seniors•At•Home division, but to people of all ages with chronic or terminal illness.
