  The name of the condition this medicine will treat
  How the medication works
  How often should I take it
  How long is it effective
  How long I need to take it
  How I know if the medication is working
  How I can expect to feel once I start taking the medication.
  When I should take it. Before or after meals? At bedtime?
  What should I do if I forget to take it
  What side effects might I expect
  Should I report any side effects?

Interactions with other medications, including over the counter meds.
  Options if I chose not to take it
