"I would like to comment that I was satisfied completely with the [home care] services you provided as well as the ladies you sent. Each one was professional, caring and compassionate. I especially appreciated their understanding at this difficult time for me."
"The Seniors·At·Home social worker assigned to my case did everything in her power to help me."
"The woman you sent recently attended my husband and I so very much appreciated her affection, professionalism, compassion. Though I did not want her to do anything but take care of my husband, she found other things around the house that needed doing, and did them."
"I appreciate how quickly and well the intake staff was able to set my mother up for immediate care after her accident."
"[Our Seniors·At·Home home care worker] was kind and loving to my dad; she talked and joked with him and comforted him when necessary. She was conscientious and patient in seeing to all my dad's needs. I felt very secure when [she] was present."
What the Industry Experts Say
Seniors·At·Home is the recipient of the prestigious national American Association on Aging (ASA) Award for Innovation and Quality in Managed Care.
Co-sponsored by the ASA and Pfizer, Inc., the award honors Seniors·At·Home's unique partnership with Health Net California, a health care delivery system. The Health Net/Seniors·At·Home partnership effectively shares responsibility for identifying, assessing, enrolling, and following up with patients who are members of Health Net Seniority Plus, a Medicare+Choice HMO, in the Seniors·At·Homecare program. Health Net care managers or medical groups identify frail seniors eligible for the program while Seniors·At·Home performs a complete assessment, recommends an appropriate plan of care, visits seniors in their homes to implement the plan and carefully reports and tracks the patients' progress and outcomes.
Launched as a pilot program in the San Francisco Bay Area, the collaboration will be expanded to Sacramento and Southern California.
